Making a Referral to Hospice of Little Traverse Bay

First, call Hospice of Little Traverse Bay at (231) 487-4825. Identify who you are and the Doctor making the referral. The following information will be required:

  • Patient Name
  • Date of Birth
  • Attending Physician
  • Hospice Diagnosis
  • Contact person's name and number

Next, fax the following information to (231) 487-4228:

  • Demographic Sheet
  • Documentation to support hospice diagnosis
  • Examples of supporty documention:
  • Most recent History & Physical
  • Most recent office visit note
  • Recent blood work, radiology tests, etc.
  • Patient's most recent Height and Weight

Once we receive this information, Hospice of Little Traverse Bay will:

  • Contact the family member identified and arrange for an information visit.
  • Follow up with your office after the information visit regarding the outcome.
